Oligoarthritis, a form of presentation of ankylosing spondylitis

Abstract

Ankylosing spondylitis is a chronic inflammatory disease that primarily affects the axial skeleton and sacroiliac joints. However, it can also involve peripheral joints, which in some cases is a challenge in the differential diagnosis with other rheumatic diseases. This case presents a young man with a clinical presentation of peripheral oligoarthritis and lower back pain who was initially diagnosed with rheumatoid arthritis. Although the coexistence of both diseases is uncommon, it is essential to consider them in the differential diagnosis. Imaging studies and laboratory tests are crucial pillars in the diagnosis of this condition. Treatment with secukinumab demonstrated significant improvement in the patient's clinical status. These new biological therapies have proven to enhance quality of life and prognosis, emphasizing the need for a therapeutic approach tailored to each case.
